Issued in a hand-screened (red ink on goldenrod paper) double-sided 6-panel folded poster sleeve. Disc labels are blank white. Catalog number derived from the matrix. In the side A runouts, OF is upside down relative to the rest of the string. In the side B runouts, 'GROUND ZERO NY △' is surrounded by small etchings which depict eyes where the iris and pupils are the anarchy symbol (capital A inside a circle). Runouts are etched. Lacquer cutting company and engineer identified by the runout etchings.
